What is Medical Case Management?

SSHC’s Case Management program provides community-based HIV/AIDS Targeted case management and health education services that focus on people living with HIV/AIDS, that have either fallen out of, or, are occasionally engaged in HIV care and treatment. The program serves PLWHA that are struggling with other chronic health issues including, but not limited to, HEP C, diabetes, heart disease, substance abuse, and mental illness.
